I noticed the same issue recently with my iPhone 13 Pro max! Ten duplicated old passwords are dated 10/9/2019. However, I'm able to delete passwords on iCloud passwords created after this date on my iPhone. My Mac and iPad don't have these old passwords. I also noted that passwords on the Mac and iPad are not in sync.
The passwords & keychain on all three devices are not in agreement. Another thing I've noticed is that all three devices (iPhone 13 Pro Max, Mac and iPad) use the same Apple ID, but have three different AppleID passwords for each device. Apple ID and the three different passwords used for each device could be what's causing this issue.
Just found solution! Go to launchpad on Mac, open Keychain Access. Highlight iCloud and scroll down until you find the troublesome websites that are duplicated. Highlight them with shift & command: then go to edit and delete all highlighted. When you close out check passwords on iPhone and they will be gone! Worked for me. Let me know if works for you.

Yes. I have tried everything to delete them, but not working. They keep reappearing in passwords under settings. Have logged out of Apple, then logged back in & still does not work. Have also unchecked keychain in iCloud, tried to delete the passwords & that doesn't work. These are 12 duplicated old sites with passwords from years ago that keep popping up just recently. I think it has something to do with the recent iPhone software update. Don't recall that this was a problem prior to that. I sync all data with my Mac laptop & passwords on laptop are fine. Can't figure out why this is just an iPhone issue. Any suggestions?
Thanks GTaylorOne! I at least know that I’m not the only one. Funny when you mentioned the date of old passwords, was mine too! There’s a glitch somewhere. Might be passwords to apple, but not sure as mine too are different, but hasn’t been a problem in past. If you gain any more info. please update. Thanks!
Which passwords? Are you referring to Keychain Password?

Very encouraged by this as I’ve been having the exact same problem. If I don’t have a Mac, is there a way that I can do it from my iPhone or logging directly into iCloud from the web?
