Two possible Options, both involve “contacting” your carrier:
First, make sure that your phone is connected to WiFi.
Then …
1) If you have an existing online carrier account, you might well be able to reprovision yourself using the online tools available.
or …
2) Call your carrier (from a different line) and request that your phone be reprovisioned.
