Images and documents have been appearing in the 'recent' folder for several weeks, even though I haven't opened them.
I have changed my iCloud password and logged in again on all devices and still this happens.
How come these files end up in 'recent' even though they are not opened?

The recent folder is a file system search results display, based on the last accessed file date.
You can use Finder -> File (menu) -> Get Info… to check the dates of files found in the recent folder.
Just a guess, could you have moved them from one folder to another?
The "Recents" folder is just that ... recent files you've opened.
The only solution to my problem with Recents is to right click the folder in Finder and then select "Remove From Sidebar". It's still in use, it just doesn't appear. You have very little control over the Recents folder in reality.
ku4hx reminded me, I don't use the Recents folder either.
Nope, did nothing with de files... they just appeared in de 'recent' folder
