Two-factor Authentication completely replaces Security Questions.

Older AppleID accounts, when upgraded to use two-factor authentication (as is necessary to access many of Apple’s most recent features) can be returned to using Security Questions for 14-days. After 14-days, previously used Security Questions are permanently discarded - and downgrade of the AppleID account becomes impossible.

All newly created AppleID accounts only use two-factor authentication.
