Hacked iPhone

I need to know if there is a way to tell if my phone has been hacked or if malware has been put on it.For example:something called “tv provider” showed up o my apps and I can’t get rid of it!If I click on it ,it takes me to a screen that says put in your subscription service(with a list that must be every network in America)so I did,and then it wanted a password etc.which I have no idea of…Has anyone else had this happen,and/or know how to remove it?Thank you!!!!
