Apple does not offer insurance. It offers Apple Care+ which is a warranty extension and has a version that offers theft & loss protection for iPhones.
You cannot move Apple Care+ from one phone to another. You can cancel the existing Apple Care+ plan for the old iPhone, and purchase a new Apple Care+ for the new iPhone.
