Hi you need more then that actually.
You will need the proof of purchase for its removal from the iphone and after that restore device.
But on the proof of purchase invoice you will need the price serial number and also the store was purchased from.
Or if you remmeber apple id or password just go to icloud.com/find sign in and remove device from account that sorts it in seconds.
