Mysterious notification of new iPad having been added to my account & compromised password.

First question:

My iPhone, MacPro and iPadPro show notification that an iPad has been added to my account, with access to my iMessage etc., but nothing has been add by me and there is no new iPad listed under 'my devices'. What's going on?

Second question:

Subsequently, on attempting to make a precautionary change the password for my Apple ID, I find my actual password, which is not in use elsewhere, is now rejected.

If my account has somehow been hacked, as now seems likely, how can I reset my password?
