Deleting an app permanently from icloud
How can I permanently delete an app from my iCloud?

Apps are not stored in iCloud. When you download a previous purchased app, it is downloaded from the App Store. The cloud icon there only indicates that you downloaded it before.
Also, while you cannot delete purchase history, you can hide it. See: Hide purchases from the App Store - Apple Support
