Hello norine145,
You came from the right support page. You need to file a request with Apple Support - start an Activation Lock support request. I'm not sure if they only accept one request or not, but you need proof the device is yours.
If the device is yours and you remember the credentials you can also just remove the Activation Lock yourself. If the device isn't yours, it will not be unlocked and you should return it to it's owner.

Yes I have submitted a request to remove the activation lock of a phone I purchase from Verizon. I sent Apple some documents but they rejected the request and I have now located the receipt and wish to resend. When I try to go through the link Apple sent it say they already have a request from that emai. Which is true because it is my original request. How do I add to original request to remove activation lock?

Contact Verizon. If they sold you a Verizon locked phone you'll have to either use the Verizon service or ask Verizon to remove the lock.
