Extensions not working

First, try a restart, as a restart can solve many issues within a device: Restart your iPhone

If you still have issues, try updating, as a new version of the iOS also corrects problems, and it may have been something Apple put in the most recent updates to correct: Update your iPhone or iPad

What happens when you try to dial an extension? What does the phone do? Shutdown? Not read the input? Not give you the keypad? Frozen keypad?

Do you have issues on all numbers that you try to dial extensions, or just that one?

When was the last time you were able to dial a phone number with an extension?

Any issues dialing a phone number without an extension?
