You would just transfer the contents of the old iPhone to the new iPhone. You do not want to create a new Apple ID. Please refer to - Change your Apple ID - Apple Support - if you'd like to change your Apple ID without creating a new one.

You can, but you will not be able to update any apps purchased with the old account. It really doesn't make sense to create more than one ID unless you have a very good reason for doing so. Read this support article about Apple ID Frequently Asked Questions --> https://appleid.apple.com/faq
