I'm not familiar with that one. The original Macintosh icons were 32 x 32 pixels, so it might be easier to design one yourself.
Maybe contact Matti Haveri who remains a frequent contributor to this site. I'll try to get his attention.

Or, maybe find some labels that suit your needs on eBay.
This is for an ancient floppy disk. So, look on eBay or put a post on craigslist.

I don’t recall an icon for the floppy eject, but there were definitely sound effects…

USB 3.5” floppy drives are readily available, older 5.25” floppy drives of various sorts are scrounge-able, but slot-loaders with host-eject functions are going to be rare. And there are floppy disk emulators, for those that need those.
Thank you all. Apparently there was no icon on the computer, it was just the sound effects. The icon I remember was created by someone just for fun. It was cute, the little happy Mac face with eyes squinched shut and tongue out.
Was it MacPuke? ...sorry, I have not used that but maybe I must try it on my Mac Plus (or SheepShaver, Mini vMac or Basilisk II).

The "sick Mac" icon would appear if it failed its power-on self test:
... but that's as close as it gets to an "official" Apple icon.
It was a little icon that came up when you ejected a floppy disk. I remember my sister getting such a kick out of it.
Getting the accompanying sound effect might be more challenging. Getting a floppy disk drive, even more so.
