The apps were fully removed. You did a factory reset so everything was deleted from your iPhone.

You would know if you had jailbroken your phone. It is a rather complex process, you couldn't do it by accident. Jailbreaking is not supported by Apple so your iPhone would not be jailbroken when it was purchased direct from Apple.

It is extremely unlikely you will find malware in any app at Apple's App Store. Everything is carefully screened by Apple, and if anything did manage to evade detection by the App Store, the millions of users would find and report it almost immediately.

The little cloud symbol you see is simply an indicator that you have already purchased that app, and you are able to download again without restrictions or payment.

A non-jailbroken phone cannot be hacked, get malware, or infected by a virus. None of your apps were infected. Feel free to reload any app that you had.

Your apps are fine. It is somewhat difficult to jailbreak a phone. Buying a phone from Apple and only getting apps from the App Store cannot jailbreak a phone.
