Hello egwuda,
Battery Health is expected to drain over time and faster the more / longer you use iPhone or any device. It is important to pay attention to apps that are consuming the most of your battery. You can find these within the Battery section. If an app is using too much, revoke background updates / refreshes. I'd also recommend turning on Auto-Lock under Display & Brightness. This will prevent your device from being on all of the time.
If you're interested in learning more about batteries and maximizing their health / lifetime
Battery health is a normal effect of battery aging. Note in your screenshot the description under "Optimized Battery Charging" and its comments about battery aging.
Engaging that option may help slow the natural aging process.
Why do you not have Optimized Battery Charging enabled on your phone? Is it your goal to have the battery decline faster than normal?

How old is your phone? Did you buy it brand new?
