Flashing folder question mark on my Mac

hello everyone.. i have a problem with a flashing folder all of a sudden, i cannot do anything s to format or erase or repair as all the buttons are greyed.. the only button i can use is verify disk which i did and its showing me a green tick.. i created an external drive and booted it from there but back to utility the left section of the internal section is greyed.. i will post some pics of what i have been thru so you all can have a clear idea.. i even tried the source to destination in the restore but its saying some 254 error or something like that.. please guide me on how to fix this, or if i can recover my internal drive if not i don't mind formatting it, but even that doesn't seem to work.. please help.. much thanks..
