How do I get reminders to vibrate or play a sound on my iphone and/or apple watch

I have been struggling with this issue since the iOS 16 update namely where my iPhone won't play any sounds associated with any reminders or alarms when my iPhone is locked. My iPhone is linked to my Apple Watch & 'mirror iPhone' is selected but any alarm or reminder I set just goes to banner notification only when my phone is sleeping/screen locked so there's no sound played, despite settings. I rely on these Alarms and Reminders being played on both watch and iPhone for my medical condition so not having them is incredibly frustrating & I'm starting to have significant negative impacts.

my reminders notifications are silent when im wearing my apple watch. how to fix this? My notifications settings are on and set to a ringtone that plays when I’m not wearing my Apple Watch, but when I put on my watch it’s silent.
