MacBook Pro charger is really hot!

My MacBook charger gets really hot; the MagSafe square part gets hot and sometimes the laptop itself, near the MagSafe plug. Sometimes it happens and sometimes not. Battery replaced a year or two ago, but this happened in the past with the old battery too sometimes.
