The ECG function is considered a medical device and your government must first approve the ECG function for use in your country. Then Apple will activate the app for Bangladesh.

The ball is in your country's court. Contact the government agency in Bangladesh that authorizes medical devices there and request its approval of the  Watch ECG function.
