You may want to check that you aren't on "mute" when making a call,
and that the volume is adjusted while you are on the call.
(Else: If needed you might want to try using speakerphone to see if that works.)

Trouble-shooting:
if you use lower power mode, you could test whether turning it off helps or not:
Use Low Power Mode to save battery life on your iPhone or iPad - Apple Support
Eg: To turn Low Power Mode on or off, go to Settings > Battery. You can also turn Low Power Mode on and off from Control Centre. Go to Settings > Control Centre > Customise Controls, then select Low Power Mode to add it to Control Centre.

If you use VPN, try turning it off, and test if there is any difference or not.

If your iPhone is not working right, try re-starting it:

Check the microphone setting is "on" in control Centre:

eg: check if a speck of dirt or wax may be blocking your voice:
