How do I use my iPad Pro as a scanner. The instructions say to tap the tap camera icon a new note in NOTE There is no camera icon nor is there a + in the right hand corner. Do I need do something in settings to activate the camera in notes?

By far the easiest method to scan a document with your iPad is to use the Files App…

From whichever folder you would like to scan your document(s), touch-and-hold (i.e., a long-touch) to expose an Action Menu. From the menu, select Scan Documents:

If you’re really still running iPadOS 16.1, update to iPadOS 16.6, and check again.
