iPad safari password utility not saving suggested passwords to keychain

I have had 5 or more times recently and further back where the suggested strong password wont actually be saved to the keychain. This means you have to click the forgot password option on the account you just created and start over. Is this a bug in the password utility?

I have also had times when it did save but failed to appear until a few days after the account was created. So the original missing password will show up in a search alongside the second one that was generated after going through the forgot password option on a website.
