Download and run Etrecheck. Be sure to give it Full Disk access before running.
Copy and paste the results into your reply. Copy the report
and use the Additional Text button to paste the report in your reply.
Then we can evaluate the report to see if we can determine the cause of the problem.
If for an app, do you have auto save activated for that app? MS Office and others have auto save features as user selectable options.
Open System Settings->General, Login Items and add/Delete what you want!

Thanks, but not seeing anything there regarding saving documents, pdfs, etc.
Where are these documents/pdfs located when you open them initially?
Emails and from any site that I go in. If I open a document to read it, it saves. It saves using the first letters or numbers of document as the name of the document.
