When trying to enable the ECG, Blood Oxygen, Cardio function on my serie 7 Apple Watch using the iPhone 13 Pro Max. Latest IOS and Latest Watch Apps update. I get an error message “Unable to confirm location. Make sure your iPhone is not in airplane mode and has a working SIM card.” I have checked both the watch and iPhone and neither are in airplane mode. Suggestions?

You didn't mention if the iPhone was online with a working SIM or eSIM, and if the Watch is GPS-only, or is cellular with its own data plan or with cellular turned off.
