Running First Aid on "Macintosh HD" Fails in Recovery Mode - (-69845) 2019 iMac Intel Based

While trying to upgrade to Ventura 13.5.2 on my iMac 2019, it repeatedly failed to update. I tried Safe Mode and same result. Failed upgrade. I then went to Recovery Mode to run "Disk Utility", as recommended by many and my computer. However, when I run "First Aid" on my Macintosh HD, or Macintosh HD - Data, they both fail. I have tried over and over. I have also tried to reinstall Ventura via the Recovery Mode but it fails as well.


I am a novice, so please be nice and patient. :-)


Any help or suggestions are greatly appreciated. I am student and I need my computer. I get can it to load to Ventura 13.5.1 but it runs for a bit, then crashes and reboots.

I followed the steps (see below) you provided. However, during Step 5, I get the -69716 Error Code. (Screen Shot Below) I am lost what to do at this point. Thank you for any help.


Intel-based Mac:

1.-Turn off Find My Mac and sign out of your Apple ID

To turn off Find My Mac, go to System Settings > Apple ID > iCloud > Find My Mac, and toggle Find My to OFF.

Then click the back button to return to System Settings > Apple ID, and click “Sign Out…” to sign out of your account.

2.-Back up your Mac

Back up your Mac using Time Machine.

3.-Start Internet Recovery

Boot to Internet Recovery (Option-Command-R or Option-Shift-Command-R) to prepare to downgrade to the version of macOS that came with your Mac or the closest version that’s still available.

4.-Erase volume group

Open Disk Utility and select “Macintosh HD”. Select the “Erase Volume Group” (not just the volume “Macintosh HD”). You may have unexpected results if you do not select ”Erase Volume Group”. When complete, quit Disk Utility.

5.-Reinstall macOS in Internet Recovery

Click on Reinstall macOS and follow the onscreen instructions. If Find My was enabled on the Mac, you will need to activate the Mac when the reinstall process completes. At the end of the activation process, you will be prompted to restart the Mac. More details about reinstalling macOS are available here.

Apple makes it very clear Betas are offered as is, and here is no support for them outside of the Feedback Assistant App. https://beta.apple.com/sp/betaprogram/submitting



The current stable release of Ventura including bug fixes, security updates is macOS Ventura 13.5.1
