Why does my iPhone show a different model when I login to other apps?

When I log into some apps it shows my iPhone 11 as an iPhone 12,1. Also why does my login show my location from coming another place in the country where I have never been? Sometimes it shows me in another state, other times it shows me in this state but won’t define the city. I have all settings to show precise location.
