Adding an emoji Beside Battery Percentage:

Unofficially, your Can't: Take a full screenshot, and then add to it using the Photos App. Then, use that as your background. There is really no other way to go about performing this. Note that in iOS17, while you are on a call, the screen would turn grey, and you would not see this in any magnet whatsoever. A bit of a d downside to iOS17, but that's the way Apple has it designed.

Do your Part: These are user-to-user forums. Apple rarely reads these forums on their own time as it is. So, provide Apple with feedback on this.  Apple wont get back to you directly, but the more they hear of this, the more they will know what their customers prefer, and what features to include in iOS versions to come.

Make this a Contact Photo:

If you were to make this into a Contact Card and assign it someone, you would see it in fill with this added image, instead of the "ugly grey screen", everyone refers to it as (on these forums). See here: Add or Edit your Contact info and Photo on iPhone - Apple Support
