The fact it worked under safe mode, says you probably have some memory resident program that is affecting caches.
The worst of these are products of Zeobit and Macpaw. Advanced Mac Cleaner, and MacKeeper. Some of them managed to get posted on the App Store, but never get negative reviews people as Apple will not let non-purchasers add commentary to the store. We have learned these products are as bad as malware.
If you suspect you might have one of these programs, look in your applications folder, and use

IMAP and Exchange mailboxes are by default stored on the server.
Smart Mailboxes need a working Spotlight index.
Rebuild the Spotlight index on your Mac - Apple Support
Check also that your mail filters aren't selected. When this icon is filled with blue it is selected:
When filtered not the entire content is shown.
If you still have nothing in the smart mailboxes, it is possible their search parameters are corrupted in some regards. Try creating new smart mailboxes for the parameters you need.
We need to see what all is running, a report from this will not display any personal info...

I forgot to mention that before I posted this question I used safe mode and the problem went away. My smart mailboxes were full.
Also I don't use iCloud for Mail and wasn't using filter. I tried to rebuild the index but nothing happened.

Apple hid the Users' Library folders...
Method 1:
1 From the Finder, select the Go menu at top of the screen, and choose Go to Folder.
2 In the window that opens, enter ~/Library, and click Go.
I was wrong. It wasn't working properly in Safe Mode. However after wasting several hours chatting with Apple support I have decided that the problem is with version 11.7.10. I created a new user and installed Mail there and the same problem occurred. So i have installed it on my MacBook which uses High Sierra and it worked so far.
I didn't cure it on Big Sur. I wasted a lot of time trying to rebuild the indexes. I gave up and installed Spark and it is working.
I have a file in Library which has Macpaw in the name and is in a couple of Gemini folders. I also have a file in Library application support which has Advanced mac cleaner in the name. I don't have any with Zeobit or MacKeeper in their name.
I'm going to delete them- see if that helps
Also if you chose to backup your email to iCloud, it will no longer be on your Mac, but rather synchronized with your iCloud account.
I just noticed that some smart mailboxes were being filled (after I rebuilt the Inbox) but very very slowly and once "filled" they are empty the next time I view them until I rebuild them again.
the smart mailboxes are working properly in MacOS High Sierra.

That was my cure 😢. I moved my email to my MacBook and will leave it there until Apple fixes it. I will use my iMac for other apps.
