Difference in the red dot and orange dot. Sometimes on a call it's red and other times orange. Am I hacked? It doesn't matter if its on wifi calling or I am using cell only. What do I do?

Not sure about a red dot, but the orange dot signifies that the microphone is being used by one of your apps. Which is normal for the Phone app.
Possibly the orange dot looks more reddish sometimes from contrast with the background color or ambient light?
