Unable to add bank debit card to wallet, receiving error unable to add card.

Sometimes if you use the camera to scan the card it's not scanning it properly especially in low light conditions, so check carefully what has been scanned and if wrong just edit it manually. If it still doesn't work because you can't activate the card then you should contact your bank and ask how to do it, maybe you need to activate it on the phone?
