I added a blank page to my PDF document. I typed out several paragraphs in a text box on the blank page and saved the PDF. When I exited out and went back into the PDF, the added page (and, therefore, all of my work) was gone. There is no saved version with the additional page in my Time Machine. Why did Preview delete the page despite the fact that I saved the PDF? Is there a way to recover my work?

Maybe this: see if the problem still happens in Safe mode (press and hold the shift key down at startup). It can take much longer to safe boot (5 min) so be patient.

Safe mode very often corrects weird software behavior by forcing the OS to do cache clearing and other housekeeping. When you're in safe boot, the machine will not be at its best performance, especially with graphics, but that's expected.
Does the problem persist while in Safe mode?
Often not required, but consider allowing the Mac to "soak" in Safe mode for an hour to allow maintenance routines and processes time to complete, then exit safe mode by restarting your Mac normally and re-evaluate the issue again.
On macOS Sonoma 14.0 and using Preview, I inserted a blank page to an existing single-page PDF. I then added a Note annotation with a large paragraph of text, and then:
In each of the above instances, I removed and re-added the Note annotation with text and saved the PDF. Under no circumstances above was the second PDF page removed by Preview. The page can be removed by the user when the thumbnail is selected and then pressing Delete.
Apple has the craziest way of selecting where a post goes, it drives me crazy enough nptto even ask questions here.
All I can suggest is try Acrobat reader...

I posted this on behalf of my friend who is using the most recent update. It made me link my old computer that I no longer use.
I was wondering if the blank page had to have something, anything added to it before the save?
Apparently not. Preview will save a blank page without deleting it, as I just did and the blank page was present upon reopening the PDF.
Preview deletes added blank pages on my PDF document.
