iPhone 11 brightness constantly changing

I have both Auto brightness and True Tone turned off but the brightness constantly keeps changing even when the phone is in active use. I see this question being asked all over but with no answers other than suggestions to turn off those mentioned above. I've tried resetting the device without using a backup, toggled the settings etc.

Have you tried a forced restart?

Press and quickly release the volume up button.
Press and quickly release the volume down button.
Press and hold the side button.
When the Apple logo appears, release the side button.

I had done that in the past and remembered it not working (before I went for a factory reset).

Tried it just now just in case and it DIDN'T work. Brightness dimmed right away. BTW, adding battery health at 84% in case that adds any more context.
