I have a hunch it is an issue with 14.1 The Ultrastudio Monitor is not seeing FCP. I spoke with the folks at Blackmagic Design and installed new driver and all is well on that side but the AV out in FCP is greyed out. Any details on this issue?

I happened to see a post on one of the BMD forums which appears to describe the same issue and a solution. It is on the macOS end related to changes in Sonomaa and the resolution is provided by Apple Support as follows:
If you can't use your camera or video output device after updating to macOS Sonoma 14.1

I've tried twice to post the solution to this but my posts are not showing. I don't know if it is because I posted a couple of links which have been blocked but I've posted links before with no problem and one of them is to an Apple Support page in any case. Anyway try searching the following text which will get you the Apple Support page and should solve your problem.

This worked for me as well. I had this exact problem, once I did the change in terminal, it fixed the problem. I'm really surprised that BMD has not fixed this issue with their driver.
When you do the recovery mode are you simply booting up and holding down Command R?
