If the paired iPhone is nearby, then it will use the iPhone's radio to make the emergency services call.
If you are at home, and you have WiFi calling enabled on your iPhone service, and on your Apple Watch, then the Apple Watch can use WiFi calling at home to make the emergency services call.
If you are away from your iPhone, and away from your WiFi, then there is nothing the GPS only Apple Watch can do.
NOTE: if you have a cellular Apple Watch BUT have not enabled the cellular service, it is possible to make U.S. 911 calls (or the equivalent emergency services call in other countries).
