How do I get rid of duplicate and missing tracks in itunes library?

Itunes shows two entries for one mp3 file and also shows one that is listed as missing.

Select the entry (for the missing song) in iTunes and right-click/Delete from Library:

yes I can do that, but that will take forever since every song is listed twice with one not available.
