You cannot.

ECG is built in, in countries that support it.

Of you are in a country that has not allowed it, or travel to one, the EKG app will not be available.

If you set up her watch using the Family Setup option, then the ECG option is not available.

See the "Availability of Features and Apps" in the following support document.

The following features and apps are not available: Medications,
respiratory rate, irregular heart rhythm notifications, ECG, AFib
History, Cycle Tracking, Sleep, Wrist Temperature, Blood Oxygen, Walking
Steadiness, Audiobooks, Remote, News, Shortcuts, and the double tap
gesture.

If you set the Watch app directly on your iPhone, then it should be available, but your data and hers will get merged in your iPhone's health App.

Phil0124, thank you for replying my question. Yes, it is ECG not EKG. However, I do not see ECG in my mother's Apple Watch. She does not have an iPhone to be paired with, so she is under my iPhone's All Watches. We are in the USA. I do not understand why her Apple Watch does not have ECG installed. Should I bring her Apple Watch to a near by Apple Store? Please advise. Thanks.
