my gfs ex had install a parental controll app on her phone but she cannot delete the app no longer what do i do?

Restore the phone to factory settings using DFU mode→How To Put An iPhone In DFU Mode, The Apple Way. Set it up as a new phone; do not restore a backup.
Then the phone has had a hardware modification or her Apple ID has been hacked or her phone number compromised. A DFU restore as described in that link completely removes all software from the phone, so if it is still compromised the only sensible thing is to destroy it and get a new phone, new number and create a new Apple ID.
ive already tried it that but it just seems like the phone has been hardwired to it?
