If your paired iPhone is nearby, the the GPS only can do everything the cellular model can do. The GPS only model will just use the iPhone’s radios.
The GPS only Apple Watch can leave the iPhone at home while exercising and track your status (walk, run, bike, etc…), as well as play music stored on the Apple Watch. When it gets back to the iPhone, it will sync your exercise data.
The cellular can leave the iPhone at home and as long as it is in range of cellular service, it can make/receive phone calls, messages, access the Internet, etc… because it can use its LTE cellular radio.
The cellular models have some different cases, such as stainless steel, etc… where as the GPS only is aluminum only.
I have had a GPS only Series 4 for the past 5 years and it has served me well, as I always have my iPhone nearby.
My wife started with the same GPS only Apple Watch, but switched to a cellular Series 6 two years later, because she did not want to carry her iPhone when walking and bicycling.
