I have a lot of personal notes on my work iphone. I will be leaving my current employer at the end of this month and want to know if there is a way I can transfer all my notes to my personal iphone.

Notes are tied to your Apple ID or an email account, not a specific device. If the notes are stored in your corporate email account, then you will need to download hem from there to get them onto another device.
If the Notes are stored in your corporate Apple ID / iCloud you can download them from https://icloud.com by logging into the website with the Apple ID used on the iPhone.
If they are stored in a personal email account, then you can just add the email account to your personal iPhone to sync them to it.
