Phone dial specific issue with extensions

When I call a number that has an extension, more often than not the extension will enter wrong. The numbers work fine to dial the number, but for example, if I need to dial extension 101, I may get a message say extension 1001 is not valid, or other times it will say extension 1101 is not valid. Even if I have the extension entered into automatic dial on a contact, this will happen some of the time. I don't have bouncy fingers, so I don't think that's the issue. I don't have accessibility voice control or voice over turned on, which apparently sometimes causes this issue.

Hopefully someone here has had the problem and solved it....
