Apple Watch VO2 max in hilly locations

My VO2 max estimation on Apple Watch was consistently around 50 until I moved to a hilly location. Now I walk and run on hills every day, and am objectively much fitter, but my VO2 max has significantly dropped by around 7 points.

Does the watch struggle to measure VO2 when in hilly or not flat locations?
