Locked SIM CARD

PUK = Personal Unblocking Key

You will need this when you enter the incorrect Cellular SIM PIN three times. If you enter the wrong PUK three times, your Cellular SIM card will be permanently blocked - and will require replacement.

You can obtain your PUK code, unique to your SIM card, from your Cellular service provider.

You may be able to find the PUK code by going to your Cellular provider website and looking up your account information. In any case it will have to come from them and is not something that Apple can give you.
