If restarting does not sort it, try a safeboot and compare your results
Use safe mode on your Mac - Apple Support login and test reboot as normal and test.
You can try from the Terminal.app Refresh the Control Strip in Touch Bar, copy & paste:
killall ControlStrip
no insight— try making change in >System Settings>Keyboard>Touch Bar Settings... make changes to Control
Strip

I will say typically it indicates a hardware failure
In or out of warranty you can get a free over the counter 'Apple Service Diagnostics' test /assessment
