You need to contact Apple Support directly by using the link in the support document you posted from.

You might need to provide:
- An image of the full front and back of your card.
- A sales receipt showing when your card was purchased.
- The serial number from your card.
Apple Support will tell you how to post send the image to them. Do not post them on these forums.
