Orange dot above WiFi strength icon
I have an iPhone 14. There is an orange dot above the WiFi icon that is on while talking on the phone. It stays on whether I use hands free or not. It goes off if I press mute

That is the microphone icon. It means the microphone is active, which is what you would expect while you are on a call. Muting the call explicitly turns off the microphone.
