Wondering if you are directing each phone call to the hearing aids:

"When you’re on a phone call, you can switch the audio to the speaker or a Bluetooth device. If you get another incoming call, you can respond or ignore it.
Adjust the audio during a call
To change the volume, press the volume buttons on the side of iPhone.
Or swipe down on the call banner, then do any of the following:
- Mute: Tap the mute button.
- Put the call on hold: Touch and hold the mute button.
- Talk handsfree: Tap the audio button, then choose an audio destination.

Seems you can use Voice Isolation on iPhone XR, iPhone XS models (2018), and later.
iPhone mini 13 was introduced in 2021 so should be fine.
Wondering which iOS version is installed on your iPhone mini 13 though ...
NB: To use Voice Isolation during a phone call, you'll need iOS 16.4 or later
(If update needed: It is always wise to back up your iPhone first:
then go to Settings > General > Software update )
When you want your voice to be even clearer during a FaceTime or phone call,
you can turn on Voice Isolation, which prioritizes your voice and blocks ambient noises
When you want your voice and all the sounds around you to be heard during a FaceTime call, you can turn on Wide Spectrum to leave ambient noises unfiltered.

Eg: says: Turn on Voice Isolation
- During your call, swipe down from the top-right corner of your screen to open Control Center.
- Tap Mic Mode.
- Select Voice Isolation
Your selection affects only the app you're using to make the call.
Voice Isolation persists for that app until you choose a different Mic Mode

As we are all Apple users, like yourself, in the community, no-one here has access to any feedback that may have been sent to Apple. Unless some-one with an iPhone mini replies here too, we have no other way of knowing for sure whether or not others have any difficulty.
