I'm wondering how to use apple pay later. Can i use apple pay later to pay an individual, therapist, or on Amazon?
If so, how? What do i need to do?

You can use Apple Pay Later to pay for anything through Apple Pay. This includes stores, websites and therapists if they have an Apple Pay compatible Terminal.
During the transaction you select the Apple Pay Loan as the source of payment by following the steps in the support document you posted from.
You cannot use it, for sending money over Messages (person to person).
Amazon does not support Apple Pay to my knowledge.
Ask your Therapist if they support contactless payment over Apple Pay.
