Tape recording icon- 2 circles connected by a straight line at the bottom

Strange icon in status bar in the middle, to the left of the time. Like the old tape recording icon - two circles connected on the bottom by a line. Status bar was also staying black. Icon would not go away. Finally did a hard reset and at restart the icon is gone. What was the significance of the icon? Why isn’t it listed on any of the articles that show what the icons mean? Was I being recorded somehow?

Control Centre icon flashes intermittently on lock screen i sometimes notice the control centre icon( I think, not sure what symbol it is but it looks like the control centre symbol) I see it flash under the top right of my lock screen. It comes down for a second then goes right back up, falls back down goes back so it kind of flashes periodically. It only happens sometimes and for sometime then it goes away. Just wondering what this means or if anyone has noticed this?

“Recording” top left I’m getting “Recording” in the left of the bar at the top. It comes on randomly eg when I opened Settings, Safari, Netflix. It is written over the top of what is already there. Its not a red screen recording either.
