My screen was broken accidentally. I have insurance. How do I file a claim?

I sometimes get a low battery warning when the phone is fully charged. Is this a problem?

What insurance?

If you had a third party company insure your iPhone, contact them for how to proceed with a claim on it.

If you have Apple Care+ (very different from insurance), set up a genius bar appointment at an Apple store or setup a Service event to send in your iPhone to get repaired. They can check your Apple Care+ status and apply the proper pricing for it to the repair.

With Apple Care+, the battery issue may fall under the coverage and net you a new device for the Apple Care+ repair price.

For third party insurers, it is up to them how they handle additional issues.
