How do I restore the clock icon on my phone

You can always find all of your apps in the App Library. This is the last page of your Home Screens.
Clock is in the Utilities folder (or at least it is in my iPhone) in the App Library. All you need to do is touch on the Clock icon to bring up the entire group, then touch and hold on the Clock icon which will then enable you to copy and move it to anywhere in your Home screens.
